The Law and all your legal affairs may change depending on where you live. Hawaii just as other States has unique laws dealing with the validity of Power of Attorney forms. Unfortunately a regular form, i.e. one form for all States and all functions does not exist, which makes it obligatory to consult a Hawaii attorney.  You should also be conscious that in some States the statutory power of attorney forms are not usable as some people have utilized them to defraud the elderly of their assets.
